Field Trip - Sack Lunches:

If your student is going on a field trip and requires a sack lunch please fill out the order form and turn into your teacher at least ONE week prior to the field trip. Form is located in the "Other Documents" section below.

 

Meal Accommodations:

Following the California Department of Education regulations, if your child requires a special meal, such as a gluten-free meal, you must have the Medical Statement to Request Special Meals and/or Accommodations  on file in the Child Nutrition Department before we can provide a different meal than what is served.  The form must be filled out and signed by a licensed physician.  Bring the completed form to the Child Nutrition Department within the OUSD District Office.  The address is 500 Dyer Street, Orcutt, CA 93455.
